Alere Initiates Voluntary URGENT CORRECTION for Use of Alere INRatio® and INRatio®2 PT/INR Monitor System

WALTHAM, Mass. – Alere Inc. (NYSE:ALR) has initiated a voluntary correction to inform U.S. users of the Alere INRatio® and INRatio®2 PT/INR Monitor system of certain medical conditions that should not be tested with the system (INRatio® Monitor or INRatio®2 Monitor and INRatio® Test Strips). In certain cases an INRatio® and INRatio®2 PT/INR Monitor system may provide an INR result that is clinically significantly lower than a result obtained using a reference INR system (laboratory method). This issue can arise if the patient has certain medical conditions or can occur if the instructions in the labelling for performing the test are not followed.

The INRatio® and INRatio®2 PT/INR Monitor system should not be used on patients with any of the following conditions:

  • Anemia of any type with hematocrit less than 30%
  • Any conditions associated with elevated fibrinogen levels including:
    • Acute inflammatory conditions (examples may include acute viral or bacterial infections such as pneumonia or influenza)
    • Chronic inflammatory conditions (examples may include rheumatoid arthritis, Crohn’s disease, ulcerative colitis, infectious liver diseases such as hepatitis, or inflammatory kidney diseases such as diabetic nephropathy and glomerulonephritis)
    • Severe infection (e.g., sepsis)
    • Chronically elevated fibrinogen for any reason
    • Hospitalized or advanced stage cancer or end stage renal disease patients requiring hemodialysis
  • Any bleeding or unusual bruising, clinically observed or reported by the patient

Patients with any of the conditions listed above should immediately be transitioned to a laboratory INR method for monitoring their INR and warfarin therapy.

In addition, healthcare professionals and patient self-testers should adhere to the following precautions in order to obtain the most accurate results:

In addition to the precautions outlined above, Alere also recommends that patients have periodic verification of their INR using a laboratory INR method. Any patient having a significant discrepant low result on the INRatio® and INRatio®2 monitor system as compared to the plasma-based laboratory INR method should immediately be transitioned to an alternative method for monitoring their INR and warfarin therapy. Significant discrepancy in INR results may lead to a delay in an urgent medical decision to reverse a supratherapeutic INR level following the established guidelines for monitoring warfarin therapy. Such discrepancies are of particular concern when the erroneous INR result is within the therapeutic range but the actual value is supratherapeutic, i.e., when the actual INR value is 6 or greater.

Alere also recommends that patients be tested to verify that their hematocrit falls within the range of 30% to 55%. Patients with hematocrit outside this range should be immediately transitioned to a plasma-based laboratory INR monitoring method.

Alere is working on an improvement to the current INRatio® and INRatio®2 meter, which will serve to mitigate further the potential for these discrepant results.

As part of its commitment to ensuring the safety of patients, Alere has reported these device concerns to the U.S. Food and Drug Administration and is conducting a thorough investigation into these events.

Safeway Recalls Safeway Select French Salted Caramel (Fleur De Sel) Premium Ice Cream

safeway-select-ice-cream-18489PLEASANTON, Calif. — Safeway is voluntarily recalling Safeway Select French Salted Caramel (Fleur De Sel) Premium Ice Cream sold in all Safeway-owned stores from November 4, 2014 through December 5, 2014 due to the inclusion of an undeclared peanut allergen.

People who have an allergy or severe sensitivity to peanuts run the risk of a serious or life-threatening allergic reaction if they consume this product. The recalled product is in a 1.5 quart package and displays the following UPC Code and Best Before Date.

Louisiana village ban on “vicious” Pit Bulls and Rottweilers lifted

My “vicious” dog.

The village of Moreauville, Louisiana recently enacted a ban on Rottweilers and Pit Bulls within their village. Their ban apparently listed them as “vicious” dogs. According to The Canine Chronicle, that ban—which took effect December 1—was repealed as of December 2. This is probably as a result of the national attention to the ban and the ire of local resident dog owners.

An article in AvoyellesToday.com, there are more than 700 cities which have regulations or bans on “vicious” breeds of dogs.

The article cites the fact that the defunct law was based on a law in another Parish of Louisiana.

In Avoyelles Parish, Bunkie has an ordinance declaring pit bulls and rottweilers as “vicious.” In Bunkie, when a dog is declared to be “vicious,” the owner must obtain an annual permit to keep the dog in the city limits. The permit fee is $50. The dog must wear a red, circular tag — kind of a canine equivalent of the “scarlet letter” — identifying it as a vicious dog. The owner must present the permit to an animal control officer upon demand. Pit bulls and rottweilers are “presumed vicious” under the ordinance and automatically subject to all of its requirements. The dog does not have to be proven to be vicious.

The problem is that Pit Bulls and Rottweilers are not inherently vicious. In fact Dalmatians can be just as vicious as the “regulated” breeds. The temperament of any dog is based on the training of the owner.

OUTRIGHT BANS VIOLATE THE CONSTITUTION

The ownership of dogs is protected by the Constitution of the United States just like real estate or any other property. They cannot be taken away from you without just cause.

Some of you will say I have never seen private ownership of anything in the Constitution. These are, of course, people who know what others SAY the Constitution says instead of studying it for themselves. People who cannot think for themselves. People who should not own dogs.

The astute of you will remember that the unalienable Bill of Rights (the first Ten Amendments to the Constitution) has a Ninth Amendment which states:

The enumeration in the Constitution of certain rights shall not be construed to deny or disparage others retained by the people.

The reason that amendment was added was that the Bill of Rights was not a popular idea at the time. It was felt that the Constitution fully protected the rights of everyone because everyone knew what their rights were. If a specific list of rights were added, the government might infringe on rights not on the list. Thus they added this amendment as a “catch-all,” in the terminology of today.

Therefore it is clear that you have the right to own any animal responsibly.

REGULATIONS ARE CONSTITUTIONAL

The regulation of animals is Constitutional. So long as those regulations do not put a hardship on the animal or owners. I would have to say that the regulation cited above is questionable because it automatically assumes the ferocity of certain breeds.

Still the government does have a responsibility to protect the rights (not the safety) of every member of a community. As Thomas Jefferson put it:

No man has a natural right to commit aggression on the equal rights of another, and this is all from which the laws ought to restrain him.

If my dog does create a vicious environment, he needs to be regulated even to the point of putting him down if needs be. In most cases, however, the owner has to be regulated more than the dog. If it is apparent that the owner is irresponsible, they owner should be banned from owning dogs. This is Constitutional. Just as a felon may not own a firearm.

Most States have a means of restoring lost rights which can be applied to dog ownership.

RESPONSIBLE OWNERSHIP

Across the country this Christmas, kids will wake up to find a puppy under the tree. In six to twelve months time that puppy will be found in a rescue shelter hoping to be loved someday. At worse, that dog will be euthanized. Even in the shelters run by the loving “People for the Ethical Treatment of Animals.”

Most will not even realize that puppy probably came from a puppy mill in which a female dog is caged and whose whole existence is to produce puppies. Most of the animals from such breeding mills are prone to physical complications which lead to euthanizing the animal later.

Owning a dog comes with responsibility just like owning a car or a gun. Just like the inanimate objects which can be a danger in the hands of the wrong people, a dog can be a danger in the hands of the wrong people. The problem with most dogs is that they are not shown the proper love. Dogs, and other animals, have emotions just like people.

Here are some tips for being a responsible dog owner.

1. Do NOT surprise your children with a gift of a puppy. They will love on the animal for maybe 48-hours before they have to start picking up the poop, feeding him at regular times, playing with him, walking him and so-forth. Never trust your kids that they will not mind doing the right thing unless you absolutely know your children. Many children will—don’t get me wrong. Just make sure that they will. Have you given them goldfish, gerbils or guinea pigs in the past? How did they treat them? Is their objective to actually have a dog to love? Or just because little Johnny down the street got one? Be prepared to take on the responsibilities if your children do not.

2. Dogs take hours a day of training. I have been chastised in the past for my “treatment” of my dog. Yet my dog is nearly always by my side and routinely loves to meet and greet visitors to our fair city. Especially children. You can spank a dog on the butt when he does something wrong, but like children you have to do it carefully as not to injure the animal or make it meaner. Especially in cases where you have to get its attention for safety reasons, such as teaching it not to just run out in the streets in front of cars. However, like children, the tone of your voice can be a major factor in training. Spanking a dog after-the-fact is essentially useless because a dog cannot understand why you are doing it.

The GOOD news is that training sessions end over a period of time. Usually about age three they will learn to sit and stay properly and obey your commands. And you can teach an old dog a new trick.

3. Dogs will tend to become protective, but not necessarily vicious. If you show your dog the proper love, he will become protective of the property and the family just like (I hope) you do for your children. You have to be aware of that fact. If you decide to get a dog from a responsible rescue shelter, in most cases that dog will become EXTREMELY protective. It is up to you to train them when to be protective and when to chill.

4. Dogs are pack animals by nature. If you get four or five—or more—dogs, you will find that they will band together. And if they get loose they will revert to that pack nature. They will fight to protect each other. It is nice to have two dogs because they have a playmate when you are not available. More than that can cause problems. It can be done, but you have to show that much more love for that many more animals. If you pack all of those dogs in your car and drive downtown, they are going to have a pack mentality. They are protective of even your vehicle. You are responsible for training a dog when it is all right to protect your property. And it IS Constitutional for a properly trained dog to protect your car. If you leave them in the heat, however, police in most cities ARE authorized to enter the vehicle, rescue the dogs and ultimately fine the owners.

5. Dogs eat. Big dogs eat a lot. It can get expensive particularly if your dog likes more expensive brands of dog food.

6. Dogs need medical attention. They need shots. You may choose to neuter them. They need regular checkups. They may choke on something they grabbed when you were not looking. All of this cost money. Health insurance is available for emergencies and regular checkups. Like any other policy read before you sign. Your veterinarian is a valuable source of information for nutrition and warnings about certain toys and products for dogs. Particularly those made in China.

7. Learn to know your dog. If your dog is very protective, you may not want to walk him down busy sidewalks. Especially where there are other dogs.

8. My dog was trained not to fight other dogs initially. He is a Rottweiler, Labrador, Malamute and Pit Bull mix. Unfortunately he was attacked three times by dogs of irresponsible owners. So I have to carefully monitor him when we approach other dogs. Most of the time he gets along. Sometimes he does not.

9. Not all people like dogs. And it is not necessarily their fault. They may have been bitten in their youth. Their parents may not have liked dogs. Whatever. It is up to you to try to monitor your surroundings and avoid such people. Most of them will avoid you. Even people who like dogs may be appropriately cautious of your dog. They do not know you or your animal. That is why I keep my dog off to the side and I tell people that he is friendly. Remember that a dog with a wagging tail does NOT mean that he is friendly. Most kids love to pet my dog when they meet him and he loves the attention.

10. Even friendly dogs jump on people. I have tried, with marginal success, to train my dog not to jump on people. He still tends to jump up on people he knows to lick their face. He also nibbles the hand of people he likes. He does not bite, just nibbles. Usually only people he knows, though. That can startle a person, however. For the most part he just loves to lick. I also know that my dog is powerful even for his size. He has knocked ME over when I was not paying attention. He has knocked a friend of mine over when he was running hard and not paying attention. I have to account for that.

11. A dog that is chained outside and left to its own devices will become vicious. Don’t believe it? Have someone chain you in the front yard in the rain and the snow. See how you feel after a couple of hours. My dog loves to play in the snow. Not so much live in it. If you cannot have a dog in your house, do not get one. Sometimes it is necessary to chain a dog for a short time, but it should not be its lifestyle.

12. STUDY THE BREED! Certain dogs do have certain characteristics. You have to learn what they are and how to train them. Any dog not shown the proper love, however, will become a problem.

I might also point out that I know the breed of dog that my dog is likely to have problems with. I rein him in when approaching such breeds. In fact I rein him in with every dog I do not know. You have to learn to read the actions of your dog.

In training your dog you have to know that YOU must train your dog. If you take him to an obedience school, or trainer or whatever, the dog may learn to follow the instructions of the trainer, but not you. A good trainer will train YOU how to train your dog.

Do not give the dog a treat every time he learns a new trick. He will then only do what you tell him in exchange for a treat. In other words, don’t train your dog to do a trick for a treat. If you are training your dog to sit, for example, make him sit and stay several times. After the completed training session, then give the dog a treat if he has performed properly.

The bottom line is that if you do not have the time for a dog, do not get one. You will only cause trouble for yourself or the animal.

Overhill Farms Voluntarily Recalls Open Nature Chile Cheese Enchiladas

ucm425650WASHINGTON — Overhill Farms, Inc. of Vernon, California is voluntarily recalling the frozen food product Open Nature Chile Cheese Enchiladas due to potential Salmonella contamination. The supplier for the organic cilantro contained in such product notified Overhill Farms that the organic cilantro may have Salmonella contamination based on routine testing conducted. The product is sold nationwide at all Safeway-owned stores, including Safeway, Carrs, Genuardi’s, Pak ‘N Save, Pavilions, Randalls, Tom Thumb and Vons.

Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.

No illnesses have been reported to date in connection with this voluntary recall.

Open Nature Chile Cheese Enchiladas are a frozen prepared dinner packaged in a box with a net weight of 9 oz. and marked with a UPC 0-7989310436-9. The product was sold at stores from Oct 9, 2014, through December 1, 2014.

Nebraska Firm Recalls Pork Belly Product Produced Without Import Inspection

WASHINGTON – Quality Meats, Inc., an Omaha, Neb., establishment, is recalling approximately 44,372 pounds of frozen pork belly product because they were not presented at the border for USDA-FSIS Import Inspection, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. This is a Class I recall since the benefit of full inspection, a possibility of adverse health consequences exists.

Product subject to recall includes:

  1. Various weight cases of “rosderra IRISH MEATS Swine Pork Belly”

The Frozen Pork Single Ribbed Bellies product was packaged March 18, 2014 through April 10, 2014. The product subject to recall bears the Ireland establishment number “IE 356 EC.” This product was shipped to retail establishments and distributors in Georgia, Illinois, and Washington where the products would have been repackaged.

The problem was discovered during a routine review using the Automated Commercial Environment (ACE) database. ACE is a web-based portal for the collection and use of international trade data maintained by U.S. Customs and Border Protection.

FSIS and the company have received no reports of adverse reactions due to consumption of these products. Anyone concerned about a reaction should contact a healthcare provider.

FSIS issues a Public Health Alert for an imported product when the country of origin recalls the product. FSIS issues a recall for imported product when the product is not presented for inspection at the U.S. border.

A failure-to-present (FTP) occurs when importers fail to present a shipment to FSIS for import inspection prior to the product entering U.S. commerce. Failure-to-present will result in the recall of the product.

Walmart receipt scam may be a hoax

10488232_10201272146525278_5503251707837464755_nWalmart receipts, such as the one shown here, are appearing on Facebook with a scam warning. The recent one I saw indicated that it was a personal experience.

The basic scam claim is that the person checked the receipt after the cashier rang up and found $40 cash back on the receipt. The cashier claimed they could not take it off and offered up the $40. The supervisor called to the scene claimed the same thing and ordered the cashier to cancel the whole order and start again. The person claimed there was $20 cash back on this receipt, but it was corrected the third time.

Hoax web sites across the Internet universally agree that this is probably a false scam claim. Walmart has been attacked by several organizations and this could be another attack.

This, however, does not mean that you should let your guard down this holiday season. You should check the receipt for any irregularities and challenge them immediately. Look for double-charges and any extra charges. Most likely you will find only honest human errors.

You should access your account regularly to see if any charges are added to your credit card that you did not make. Report them immediately to your credit card issuer.

If something like this scam were to occur, Walmart or any other retailer would likely fire the person caught, at the very least.

Sheriff of El Dorado County strips federal law enforcement of their law enforcement powers

dagostini-01El Dorado County Sheriff John D’Agostini has stripped the power of the forest service law enforcement to enforce State laws in his county according to CBS Sacramento.

In a letter they obtained, Sheriff D’Agostini wrote:

“I take the service that we provide to the citizens of El Dorado County and the visitors to El Dorado County very seriously, and the style and manner of service we provide. The U.S. Forest Service, after many attempts and given many opportunities, has failed to meet that standard.”

The Sheriff said that he took the action after numerous complaints.

Sheriff D’Agostini is also known for a January 2013 letter to Vice-President Biden over the proposed gun ban according to KRNV in Nevada. In the letter he wrote:

“The purpose of this letter is to go on record of re-affirming my oath of office and making it clear that I and my staff will never violate [our oath of office] by being pressured into enforcing any unconstitutional provision, law or executive order.”

He noted that disarming law-abiding citizens would not prevent guns from falling into the hands of criminals.

Recall of Northstar Label Gabapentin Capsules

ucm424493Dayton, NJ, Aurobindo Pharma USA is voluntarily recalling lot GESB14011-A of Gabapentin Capsules, USP 300 mg 100-count bottles to the consumer level. The product lot has been found to contain some empty capsules.

Empty capsules could result in missed dose(s) of gabapentin resulting in adverse health consequences that could range from no effect, short term reduction in efficacy, short term withdrawal effect, or status epilepticus (long period seizures) that could be life-threatening. Aurobindo Pharma USA, Inc. has not received any reports of adverse events related to this recall to date, but has received four complaints for empty capsules.

Gabapentin is used as in the treatment of epilepsy and for the management of postherpetic neuralgia (pain after shingles). The affected Gabapentin lot is GESB14011-A, Expiration 12/2015 and is packaged in 100-count bottles, NDC 16714-662-01. The product can be identified by the imprint D on yellow cap and 03 on yellow body with black edible ink. Product was distributed through Northstar label to retail outlets nationwide.

Aurobindo Pharma USA, Inc. is notifying its distributors and customers by recall letters and is arranging for return of all recalled product. Consumers, distributors, and retailers that have product which is being recalled should stop using, distributing, or dispensing the affected lot and return to place of purchase.

Great American Appetizers, Inc. Recalls Hyvee Mozzarella Cheese Sticks Due To Undeclared Soy Flour, Yellow #5, And Yellow #6

640px-ucm423215Great American Appetizers, Inc. of Nampa, Idaho is recalling 662 cases of HyVee Mozzarella Cheese Sticks because they may contain undeclared soy flour, yellow #5 and yellow #6. People who have an allergy or sensitivity to soy, yellow #5 or yellow #6 can run the risk of serious allergic reaction if they consume this product.

The HyVee Mozzarella Sticks were distributed to retailers in Iowa, Illinois, Kansas, Minnesota, Missouri, Nebraska, South Dakota, and Wisconsin.

The HyVee Mozzarella Cheese Sticks affected are in an 8 oz. (227 g) carton (UPC #075450149913). The product date codes are printed on the end of the carton: 14290402 (BEST IF USED BY 4/17/2016) and 14295402 (BEST IF USED BY 4/22/2016). No other date codes are affected by this recall.

No illnesses have been reported to date.

This incident was discovered in-house by the company’s internal packaging review. Great American Appetizers, Inc. staff determined that the new carton did not accurately describe all allergens present in the product during the packaging update.

This recall is being made with the knowledge of the U.S. Food and Drug Administration.

B&G Foods Issues Allergy Alert On Undeclared Peanut And Almond In Product

B&G Foods announced today it is voluntarily recalling certain Ortega Taco Seasoning Mix, Ortega Taco Sauce, Ortega Enchilada Sauce and Ortega Taco Kit products and certain Las Palmas Taco Seasoning Mix and Las Palmas Taco Sauce products after learning that one or more of the spice ingredients purchased from a third party supplier contain peanuts and almonds, allergens that are not declared on the products’ ingredient statements. People who have an allergy or severe sensitivity to peanuts and almonds run the risk of serious or life-threatening allergic reaction if they consume these products.  There is no health risk associated with these products for individuals without an allergy to peanuts or almonds.

The recalled products were distributed in retail stores and foodservice outlets nationwide.

This recall does not apply to any other sizes or varieties of Ortega Seasoning Mix or to any Ortega Seasoning Mixes in canisters, all of which are correctly labeled.  For example, this recall does not include Ortega Fajita Seasoning Mix, Ortega Fish Taco Seasoning Mix, Ortega Chili Seasoning Mix, Ortega Chipotle Seasoning Mix or Ortega Burrito Seasoning Mix, all of which are correctly labeled.  This recall also does not include any Las Palmas Enchilada Sauce, which is correctly labeled.

This recall was initiated in consultation with the FDA after it was discovered that ingredients from a single supplier used in the affected products were contaminated with peanut and almond allergens. B&G Foods has since terminated its relationship with this supplier and is receiving these ingredients from other sources in anticipation of resuming production shortly.
